1. Enroll in the Loyalty program using the form at the bottom of this page.* Complete the form and return it to us.
2. Purchase a minimum of 5 cases every three-month, quarterly period. Visit our website for a list of products included in the program and their rebate amounts. (Note: The qualifying products will change every quarter.)
3. Provide us proof of purchase for the qualified products.
4. Receive a rebate check every quarter (max. payout per quarter is $125 per customer location) for qualifying products.
